We offer a safe, healing space for individuals to recover after hospital stays when they have no place to go. Our team provides support, rest, and stability during this critical time of healing.
Housing Trio
Our Housing Trio program helps individuals find, secure, and keep stable housing through a three-part approach: Housing Transition Navigational Services, Housing Deposit Assistance, and Housing Tenancy and Sustaining Services. We guide each person through every step—from searching for a place to move-in support and long-term stability—because everyone deserves a place to call home.
Short-Term Post-Hospitalization Housing
Our transitional housing helps individuals stabilize after discharge, reducing the risk of returning to the hospital—or the streets. We provide compassionate care and a dignified place to recover while planning for the next step.
Enhanced Care Management
For individuals facing complex medical, behavioral, and social challenges, our Enhanced Care Management program offers personalized, whole-person support. We build trusted relationships and coordinate care across hospitals, mental health providers, housing programs, and more—so no one has to navigate the system alone. Our care managers act as advocates, connectors, and companions, helping people access the resources they need while honoring their goals and dignity.
